LIFE AND AD&D INSURANCE Trilogy provides eligible full-time employees Basic Life and AD&D insurance, either $15,000 or one times basic annual earnings (up to a maximum of $100,000), at no cost to you. Your total benefit is reduced by 35% at age 65, 60%atage70,75%atage75,and85%atage80. VOLUNTARY LIFE INSURANCE Voluntary coverage can be purchased for you and your eligible dependents without answering medical questions up to the Guaranteed Issue amount if elected upon hire or upon gaining eligibility. Your cost for coverage under the Voluntary Insurance plans can be found in myadp.com. If you apply for coverage after your new hire enrollment period, you will be required to answer medical questions before Guardian will approve your requested coverage amount.
